Governor’s Challenge Priority Area Two

Promoting Connectedness and Improving Care Transitions

We are working so that no veteran, no service member, and no family is left behind. That means prioritizing connections, collaborating amongst many sectors, and taking a deep dive in care transitions. The below programs are examples of resources to help our veterans and military members so they are not struggling in silence.

Together With Veterans

Together With Veterans (TWV) is a community-based suicide prevention program for rural Veterans. TWV involves partnering with rural Veterans and their communities to implement community-based suicide prevention.

Together With Veterans uses a five phase process to support rural communities in developing a local Veteran suicide prevention action plan.

Phase 1: Build Your Team

Phase 2: Learn About Your Community

Phase 3: Teach Your Team

Phase 4: Plan For Action

Phase 5: Follow Your Plan and Measure Results

ETS Sponsorship Program

Transitioning from the military back to civilian life can be challenging. But there are resources in place to help. The Expiration Term of Service (ETS) Sponsorship Program and its public-private partnership with the U.S. Department of Veterans Affairs (VA), ensures that service members exiting the military don’t need to do it alone.

By working with the Department of Defense, the Department of Veterans Affairs, the Department of Labor, local experts, and local governments the ETS program has one common mission: to securely provide the information and relationships needed to help every transitioning service member successfully make the journey out of the military and into civilian life.
